Unpackaged disposable plates and an open box of foil were found in the storage area, while food prep areas displayed improper storage practices, with plates and containers not stored upside down. The floor perimeter of the walk-in cooler contained debris, and several surfaces, including the catch-basin below the food prep sink, were found to be soiled.

Most concerning were the signs of pest activity. Inspectors documented live and dead cockroaches in various locations, including the mill room and food prep area. The presence of flies in the brewery and food preparation areas raised additional alarms. A gap in the brewery’s rear door and open garage doors potentially contributed to the pest issues.

The report indicated that the chlorine chemical sanitizer levels in the dishwasher were significantly below the required range, measuring between **0 to 10 ppm** instead of the mandated **50 to 100 ppm**. Furthermore, quaternary test strips were expired, and there was a spray bottle without a label, creating confusion for staff.

Notably, an employee was observed using the handwashing sink to wash utensils at the point-of-sale area.

The inspection report highlighted improper food storage practices, with raw animal foods placed above ready-to-eat items in both the reach-in and walk-in refrigerators. Additionally, food in the walk-in cooler was stored directly on the floor, violating health regulations that require items to be kept at least **6 inches** off the ground. Dust and debris accumulation in the basement further indicated a lack of cleanliness.

Physical evidence of insect activity was documented within the kitchen and basement storage area. Inspectors found a bag of rodent bait and an insect control device that posed a risk of contamination to food and utensils. Ice buildup in the chest freezers and the absence of a backflow prevention device on the main water supply line were also noted.

The report indicated that the combination handwash and garbage grinder in the food prep area was not properly sealed, contributing to sanitation concerns. The **3-bay ware wash sink** also displayed inadequate quaternary ammonium concentration levels, measuring at **0 ppm**, contrary to the required **200 to 400 ppm**.
