UPDATE: The suspicious death of Charles Frederick Gill, an 88-year-old resident of RSL LifeCare’s Ada Cottage in Kandos, is now under urgent investigation following a fire incident on the night of August 11, 2025. Authorities have confirmed that the case has been referred to the coroner after Mr. Gill succumbed to what is believed to be smoke inhalation.
The fire broke out shortly after 11 PM while Mr. Gill was in his room. RSL LifeCare reported that staff responded within seconds, initiating emergency procedures and evacuating residents in accordance with established fire protocols. Despite their swift action, Mr. Gill was transported to Mudgee Hospital and tragically passed away the next day, August 12.
In the aftermath, NSW Police established a crime scene and began an investigation, awaiting the results of an autopsy. As of August 14, police officially reported the death to the coroner, who is currently waiting for a detailed evidence brief from investigators before proceeding with coronial inquiries.
RSL LifeCare has expressed its commitment to transparency, stating that they promptly reported the incident to both the Aged Care Quality and Safety Commission and NSW Police. Initial findings from police investigations indicated no faults in electrical systems or fire prevention measures at the facility.
In a significant development, RSL LifeCare commissioned an independent forensic fire expert to further analyze the incident. The expert concluded that the fire was most likely caused by a static discharge from a pressurized air mattress used in Mr. Gill’s clinical care—a rare and troubling cause of fire, according to the organization. RSL LifeCare has since reported this finding to the mattress manufacturer.
The organization has maintained open communication with Mr. Gill’s family and is fully cooperating with the coroner’s ongoing investigation.
